this message keeps appearing when he enters the server, but he isn't. In fact, he is not that good at all. I tried removing him from the follow database at least a dozen times now by typing !unfollow husch and the server responds that he's removed from the follow database but the next time the same message will just appear again.
It's very annoying and very very distracting if you ask me. Is there a way to fix the unfollow command on B3? Or is the command fine and am I missing something? Because it just doesn't appear to be working...

yes, I do as well, but in that case it's better to disable the feature; it's distracting as it is, if it's a new player I must spec him...can't resist and it's never true. So to avoid the message again I unfollow them so nobody else gets annoyed by the message but the unfollow doesn't work...kinda a viscious circle huh? :p

I put the explanation on the new B3 features forum here b3-bot/b3-v1-3-features-and-commands-t2092.html
To sum up, your GUID, the number identifying you, has a particular format, and if the GUID of someone connecting is different from that format, mods and admins get the warning. Only problem is, when someone connects with an old Q3 version, the IP is used instead of GUID, and that's a normal behavior, so don't worry if you see an IP address. The common format for GUID is 32 hexadecimal characters.
